3120.3 Physical Examination

Newly employed teachers and teacher aides are required to have an Employment Physical Examination as per Section 118.25(a) of the Wisconsin Statutes. An employee may wish to have this examination completed by a physician of their choice. CESA 5 will pay an amount up to fifty (50) dollars for the examination. Payment will be made directly to the physician's office upon receipt of invoice and the completed Physical Record of School Employee Examination form. If health maintenance organization (HMO) to which the employee subscriber pays for this examination, the agency will bear no additional expense. This form is available from the CESA 5 Business Services Department.
